Outpatient Continuing Care

Tracey’s Place of Hope also provides outpatient continuing care for adolescent girls with substance abuse issues (as well as co-occurring disorders) who are transitioning to the community from residential or institutional care and need ongoing support to maintain stability and sobriety. The service is targeted for the criminal justice population, youth in the dependency system, and emancipated youth.

The goal of continuing care is to assist these girls to form another, more appropriately supportive community - one in which the challenges of their recent release can be discussed and addressed.  Most clients are returning to the communities in which their substance use and other self-destructive behaviors originated; therefore we utilize a therapeutic relapse prevention and harm reduction model that helps to avoid or quickly tackle the pitfalls of early recovery and reintegration into the community.  These services include both didactic and process oriented groups, family support, integrated case management and individual sessions.  The therapeutic model is consistent with our residential program – that of gender-responsive, strength-based, and developmentally appropriate treatment.

"I thought I was going to rot in psych wards or jails. Tracey’s Place has given me a second chance. I’m not going to rot." - Tracey's Place of Hope resident, 2002
